- There is someone's shadow in this frame. The Crown Killer's maybe?
- You can hop back on the railcar and choose to go back to Campo Seta District, but the railcar will break down. The guards mentioned the engine breaking down in this note in the previous mission.

- Addermire was a solarium for the rich until Hypatia turned it into an institute for infectious disease that treats everyone. That's probably why wooden ramps were added to the original stone staircases to make them accessible for disabled people, particularly those who suffered from mine accidents. In the screenshot, you can also see a wheelchair at the top right corner.

- There is a guard called Valiente who was killed and mutilated by the Crown Killer. His head and left leg were missing. If you go to where the Crown Killer is, you can actually find a head there, along with many body parts of other victims. No way to confirm it's Valiente's head but he is the only corpse in the level without one. Arkane also created a little crime scene as seen in the screenshot. You can deduce that he was attacked outside, dropped his hat and lantern, then either ran or was dragged into the room, and killed.
- Granny Rag was once a patient in Addermire. You can destroy the two turnbuckles on top of the elevator shaft to drop the elevator down and reach a flooded basement. There you can find her diary and some other details.

- According to the Heart, Vasco graduated top of his class, and was recruited by Hypatia to be her assistant.
- It's possible to save Vasco by sedating him with sleep darts or chloroform bottles and then looting his body for his safe combination. Hypatia will still turn but won't attack Vasco. The Heart, however, says that he will not see his family again, suggesting that he will succumb to his wounds and die anyway.

- According to the Heart, Hypatia was a student of Sokolov and Piero.
- It's possible to cure Hypatia without her turning. To do this, you need to brute-force Vasco's safe (trying out each combination one by one) to get the cure, then directly use the serum on Hypatia. In this scenario, Vasco will already be dead by the time you reach him. You can see a video of the outcome here. - Hypatia's rambling gets more unstable and violent as she goes on.
- Looking at Hypatia or the Crown Killer through Dark Vision, you can see an orange cloud surrounding her. This represents her scent detection range similar to what the wolfhounds have.
- If you possess Hypatia, you will hear the Crown Killer inside her head says: "Do you like it inside Hypatia as much as I do?" or "I wonder, will this drive her insane, playing host to both..." After possession ends, she will immediately turn. Subsequent possessions of the Crown Killer yield the following lines:
- "Get out!"
- "Being inside my mind would drive you mad!"
- "There are two minds here already. No more!"
- The Crown Killer will ignore Mesmerize's void spirit, and say the following lines:
- "I sense an Ocean of blood and a black sky. But I have more pressing matters."
- "Why would I listen to those sweet whispers from beyond?"
- "What is this ugly thing and how is it here?"
- "No, I won't heed you. My mind is altogether different."
- "If I gave myself over to this, I'd lost, lost, in a dark place, gnawing bones."

- There are two maps with markers on them, possibly showing the locations of her targets. The marker next to Dunwall Tower is very likely Ichabod Boyle. Also for some reason, there is an assassin's blade there. Simon, the editor of the Dunwall Courier, also has a Dunwall map with the same markers, because he was writing articles about the Crown Killer and the victims.
- You can kill Meagan by disabling the watchtower, waiting for her to arrive then reactive it. This results in a gameover, of course. This is also similar to how you disable the floodlight in The Royal Physician mission, wait for Samuel to arrive, and reactivate the floodlight.
